   Are you using any of the software and/or switching companies listed?  
   
   
   • If the answer is NO, then we are unable to capture your data, and your pharmacy will not be able to participate in the Rx$hare program.
     
     
     
2. If you’ve answered YES then fill out and submit the Rx$hare Enrollment Form.
   
   
3. After submitting the Enrollment Form, a page will be presented for you to sign and FAX to Pharmacy First.
   
   
   • The required documents will be automatically generated for your pharmacy and mailed to you. Place your signature where required and fax those pages back to Pharmacy First.
